Congratulations on your new job. As an employEE engaged in a Co-EmployER relationship it is important that you understand your rights and what is expected of you as a new employEE. I recommend you bookmark this page for future reference, print all of the following forms and put all this information in a binder for future reference.
Give a completed packet to your Co-EmployER. They will review the packet and then forward it to TSM.

| Criminal Background check release (Release Authorization) |
A criminal background check will be run on all individuals hired by Tracy Stein Management Services, Inc. (TSM). No applicant will be denied employment solely on the grounds of conviction of a criminal offense. The nature of the offense, date of the offense and surrounding circumstances and relevance of the offense to the position(s) applied for may, however, be considered.

This form is for your taxes.

Your paycheck cannot be processed if you have not completed this form. If you want to change your status or withholdings you must complete a new form and mail it to TSM. |
| Department of Justice I – 9 form |
Department of Justice Employment Eligibility Form

You must complete Part 1 and give it to your co-employER (Participant/Parent) along with copies of your US employment eligibility documentation. In most cases this will be a copy of your Social Security card and photo copy of your driver’s license. For a complete list of documents you can use see page 3 of the I-9 form. You must sign section 1 and return to Co-employER along with your documents for review. Your pay check can not be processed without this form and the required documentation. |
| Confidentiality Agreement |
This is a confidentiality agreement between you and your employer.

| On Duty Meal Period Agreement. |
This is a meal agreement between you and your employer.

This form should only be signed and returned if you agree to it. |
| Direct Deposit Application |
This is an optional direct deposit application so your paycheck can be deposited directly into your bank account if desired.

Complete this form and attach a VOID check or bank specification sheet from your bank if you want your check to go direct deposit. It may take two pay periods to establish your direct deposit. You will continue to receive a paper check until your direct deposit has been established. Return to Co-employER. |
| Mandated Reporter |
California law REQUIRES certain persons to report known or suspected abuse of dependent adults or elders. As an employee or volunteer at a licensed facility, you are one of these persons — a ';mandated reporter'.

You must read this form and sign this form. It is required that you complete this form and return it to your Co-EmployER. |
| Payroll Processing Calendar |
Pay periods are semi-monthly. This calendar shows you all of the paydays and what days time sheets must be turned in for each pay period.

We encourage you to use the direct deposit services offered by TSM and your Co-employER. If you choose to have your check mailed they will be mailed the day before payday. |
| Time Sheet |
This is the timesheet where you record your hours.
|
It is your responsibility to get your time sheets to your Co-EmployER in enough time to have them reviewed and sent to TSM. Your Co-employER will fax them to TSM for processing.
You should keep one time sheet to use as a master.
Time Sheet must be reviewed and signed by Co-employER before they are faxed to TSM.
If I receive an incomplete time sheet your payroll may be delayed.
If time sheets are not received by due date and time your check will be delayed until the next pay period.

This form is used to notify me of changes with respect to things like your Address, Name or to be removed from Direct Deposit.
