WHAT IS TSM SELF DIRECTED SERVICE?
Tracy Stein Management Services, Inc. (TSM) will manage and issue ongoing monthly expenses based upon an approved budget and plan. If you desire, TSM can
also serve as your employer agent. To become your employer agent, you will need to complete the co-employer forms in the self-directed services intake packet. It is
important that you complete IRS Form 2678 and two IRS Form 8821 as requested.
Once you've signed up for the service and provided TSM your approved budget, the process is simple. You provide:
- A completed TSM Check Request Form detailing the expense/service information
- Send a completed check request to your service coordinator for approval.
And TSM does the rest:
- Issues checks within 3 working days after receiving an approved TSM Check Request Form/Bill by U.S. mail, fax, or
email (there are no limits on the number of checks a consumer can request)
- Watches your budget: Our system does not allow us to issue a check if the consumer does not have the available funds in that category of their budget. We notify everyone involved
if we are unable to pay a request/bill we receive
- Issues vendor required IRS W-9 and annual IRS required 1099 forms for any vendor paid greater than $600.00 a year
- Maintains all documentation for a minimum of five(5) years
- Issues monthly reports to Service Coordinator, Broker and Consumer/Family member

Monthly Reports
TSM Provides both the Consumer, Broker and Regional Center with monthly reports (Budget Summary, Budget Master, Actual Expenses and Monthly Status) of consumer’s
account with us. Reports are included in the prices of both Co-Employer and Bill Paying Services.

Budget Summary: summarizes total budget
year to date by goal/category.

Budget Master: provides monthly detail
on the amount of money budgeted for each goal/category

Actual Expenses: provides report of expenditures budget year to date.

Status for Account: provides detail
report of expenditures for the month (much like a check register).
